Wellbeing Team
Our PCN Wellbeing Team consists of Health Coaches and Social Prescribing Link Workers who provide holistic and personalised support to individuals, their families and carers, helping them take control of their wellbeing, live independently and improve health outcomes. Patients will set their own goals and will be given help to do this if needed if they struggle to identify goals. Patients will then be reviewed at each subsequent session. If you feel you could benefit from this type of support, please complete the below form so that a referral can be generated. You will then be contacted by a Care Co-Ordinator to arrange an appointment.
