New Patients

(Please allow up to 7 working days for new registrations to be processed. After this time you will be able to request appointments/prescriptions if needed. Your medical history from your current GP can take longer to be transferred, and we recommend that you have a minimum of 2 weeks supply of any regular medication from your current surgery before registering with us. .)

New Patient Registration

To register as a new patient You can either Complete your registration on line by clicking here or call in to the surgery to complete a registration form. Please bring two forms of identification to complete the registration process.

This can include

  1. Photographic ID or birth certificate.
  2. Official document containing your current address. 

Should there be a reason why you are unable to provide identification, please discuss this with our reception team.

Children aged 15 and under.

To register a child at the surgery, the parent/guardian must be registered her already or applying to register here at the same time. The registration process is the same as for adults, although children do not need to provide ID.

  • Call into our Surgery during opening hours, remembering to bring relevant Identification ( please can we also advise that anyone aged 16 or over must attend and can not be registered by another adult.)
