(Please can we advise once we have all your forms and identification it takes a minimum of 4 working days to record your basic details on our system as a new patient and at this stage you can contact us to make appointments. Your medical history from your current GP’s can take a further 7-10 days to be transferred so we strongly recommend that you ensure you have a minimum of 2 weeks of any regular medication from your current Surgery.)
Individuals aged 16 and over.
If you live close by then we are more than happy to register you as a patient here at St Catherine’s GP Surgery.
Anyone wishing to register with us needs to complete an NHS Family Doctor services registration form which will ask about your current address, previous address and previous GP Surgery details amongst other general details known to you.
We also have our own New Patient Health Questionnaire for you to complete and this will enable us to get important health history from you before your records arrive from your old GP’s.
Finally we ask that you show us two forms of Identification when you come to register:-
Should there be a reason why you are unable to provide any Identification then please call into Surgery to discuss further with our Reception team.
Children aged 15 and under.
To register a child at this Surgery, the parent/guardian must be registered here already or applying to register here at the same time as the child.
Again the parent/guardian will need to complete an NHS Family Doctor services registration form with the child’s details along with our New Patient Health Questionnaire specifically for children.
We do not need Identification for the child.
