Patient Access for appointments and repeat medication requests:

Patient Access online allows you to

  • request your repeat medication
  • view your clinical records from the date your access was granted

What happens next?

  1. Reception will process your request whilst you wait.
  2. You will be given log on details and a list of suitable providers on the internet
  3. At home you can choose which provider you want to use and then register.

Please can we advise that the prescription request element of Patient Access online applies to your repeat medication only BUT there is a free text box when ordering where you can add to the request. Also if you are unable to reset your password online at anytime then we can at the Surgery reset it for you BUT you will need to come in with photographic identification.

Patient Access for online view of medical records:

Patient Access for online view of medical records allows you to view

  • Results e.g. blood pressure, blood tests, records of X-rays
  • Vaccinations and immunisations
  • Medications
  • Letters to and from the GP (Hospital letters will only be available from 1.3.17 to present date)
  • Allergies, active problems
  • Consultations
  • Examination dates, investigations and outcomes

What happens next?

  1. Reception will check the form and copy your identification documents.
  2. You will be contacted at a later date with log on details that will give you access to your medical records as detailed above.

What is proxy access?

You can request to act as a proxy for children, relatives, and dependants that you care for using Patient Access.
This enables a parent, family member or carer to act on behalf of the patient with their access tailored accordingly.
Appointment booking, repeat medication requests, messaging and access to medical records, can easily be accessed by the proxy once a relative or person has been linked.

Requesting proxy access

You can request proxy access at the reception desk who will ask that you kindly complete the relevant form and show 2 forms of appropriate identification.
Suitable ID:

• Birth Certificate
• Driving Licence
• Passport
• Paid utility bill
• Bank/building society card statement

The forms will require both the patient and the person requesting proxy access to be present and to sign the appropriate forms and present suitable identification.

We do not require the patient to attend if they are housebound, but we still require identification and the form to be signed where possible.

If the patient is under the age of 12 then no identification is required but a signature is still required if possible.

Proxy Access For Children

For Parents/ Guardians who have proxy access for children who are approaching the age of 16 will receive correspondence informing them that proxy access may be removed and their child will need to re-register for their own access.
