Day 8 and onwards:-

Statement of fitness for work (commonly known as a Sick note or Fit note)

If you are unable to attend work for 8 or more consecutive days then your employer may ask you to provide a Statement of fitness for work.

If your absence is following a procedure in hospital (other than A+E) please contact your consultant who is responsible for issuing your statement for the duration of your expected recovery. Should you feel at the end of this recovery period you are unable to return to work then please submit your request to us using eConsult and selecting the administration option.

If your absence is for any other reason then please submit your request to us using eConsult and selecting the administration option.

Important Information:-

  1. For a continuation sick note please may we advise the earliest you can request is the last working day before your sick note expires.
  2. Please do not contact us after sending your eConsult as you sick note will be ready within two working days of your request UNLESS we contact you to pass on the GP's comments.
